All parish registration information is confidential and will only be used for the purpose of registering you and/or your family with the St. Matthew Catholic Church Faith Formation Programs. We do not and will not share your information with any other individuals or businesses. This is our pledge to you!
Simply download and print the required Registration form(s). Once you've completed the forms, please return it on a scheduled registration day. *Printed registration forms will be available at registration. We look forward to getting to know you and serving you in the future!

We need copies of the student's Birth Certificate, and any Certificates of Catholic Sacraments already received (Baptism, 1st Communion, Confirmation, Matrimony). If baptized in another Christian faith, please provide a certificate of Baptism along with your Birth Certificate.
